IMPORTANT, please read this thoroughly: The Foundation's initial review focuses solely on the hardship that created the need for assistance—not the bill that needs paid.
Please provide a detailed explanation of the unexpected event or circumstance that led to this request. An example of the level of detail expected is provided below. Referrals that simply state "rent," "utilities," "medical bills," or other expenses without explaining the underlying hardship will not be considered for further review.
Applications that demonstrate a qualifying hardship may be invited to complete an extended application. Due to the volume of requests received, the Foundation will not follow up to request additional information or clarification. Please ensure all information is complete, accurate, and detailed before submitting.
Next Steps
Step 1: Submit Application and Documentation
Complete the initial referral form on behalf of the client or family you are working with.
The Foundation's first review focuses solely on the hardship that created the need for assistance. Please provide a detailed explanation of the unexpected event or circumstance, including how it has affected the household and created the financial need.
Step 2: Wait for Review
The committee will review the hardship information provided to determine whether the situation appears to meet the program's eligibility criteria.
Please note that the initial review process typically takes 7–14 days. Applications are reviewed year-round and evaluated every 1–2 weeks. Delays occasionally occur but can be avoided by submitting complete, accurate, and detailed information.
If the hardship appears to meet the program's criteria, the Foundation will contact the referring organization via email with instructions to complete an extended application and provide supporting documentation.
Step 3: Complete Extended Application (If Invited)
If the hardship is approved for further consideration, the referring organization will receive a link to complete a more detailed application and provide any required supporting documentation.
If any information changes after submitting the referral, please email hardship@mcfohio.org as soon as possible. This includes changes to the applicant's circumstances, the amount requested, or situations where assistance is no longer needed.
